1 Varicella (Chickenpox) - History of desease is acceptable, parent must sign verification statement on school immunization record.

A student entering seventh grade

A student must have the above immunizations AND

1Tdap booster

* DTP/DTaP/DT - Only four doses are required if fourth dose was admnistered on or after the fourth birthday
** Polio - Only three doses are required if the third dose was administered on or after the fourth birthday.

The timing and spacing of vaccine doses are two of the most important issues in the appropriate use of vaccines. Useing an accelerated vaccine schedule may be necessary for children who have fallen behind schedule and need to be brought up-to-date quickly. Accelerated schedules should not be used routinely.
